The Malina portal is a digital tool that speeds up and streamlines the transition of a patient from hospital to post-acute or home care. It helps families, hospitals, and providers find a suitable facility without lengthy calling around and uncertainty. The goal is to shorten time, reduce stress, and prevent errors that could endanger the patient.
Why the Malina portal was created
After discharge from the hospital, families often do not know where to place the patient, and finding options takes days to weeks. An incorrectly chosen facility can worsen the health condition, lead to rehospitalization, and unnecessarily burden emergency services and the budget. Malina puts the patient at the center while also considering the needs of the family, the hospital, and the provider. It offers a proven, clear, and unified process designed to prevent mistakes and speed up decision-making.
How it works: modules and patient assessment
Since 2018, a public module has been available in which facilities can be searched by preferences or health status; if the provider updates the data, the system also shows available spots. A new addition is a digital LTC module: the form assesses the patient's condition and recommends the appropriate type of long-term care facility. This form has been tested since 2018 in five hospitals, where a decision algorithm was developed and validated based on it. The result is available in printed and electronic form so that hospitals and families can work with it immediately.
Results and what's next
Thanks to a unified process, Malina can shorten the patient's path to a suitable facility from approximately 30 to about 3 days, saving time, energy, and finances. The tool supports a standardized process for arranging post-acute care and is free for all social service providers. Free testing of the LTC form is also underway in selected hospitals, complemented by practical demonstrations of its use. The team is also preparing an EPD module intended for nursing care documentation in social service facilities.
